General requests for PPE need to be submitted electronically. Starting 5/26/2020 please use this link (not for lab materials). Requestable items through this link include: disinfectant wipes, cloth masks, face shields, gloves, goggles, gowns, hand sanitizer, N95/KN95 masks, surgical masks, thermometers. For all other non-testing supply or PPE requests not listed above, please use WebEOC or call the ESF8 desk at 603-223-3729.
Vermont Department of Health instructs health care providers who are anticipating a depletion of any COVID-19 specific resource stocks within the next 7 days, to submit a PPE and Test Kit Request Form.
