Resources for Homeless Populations


Community Health Centers of Burlington in Vermont started the Homeless Health Care Program (HHP) in 1989, and works with partner organizations that help treat and care for all of the homeless and risk patients in the area.


The Vermont Coalition to End Homelessness offers resources, shares information, provides a forum for decision-making and promotes decent, safe, fair, affordable shelter for all.


The Health Care for the Homeless program of Manchester conducts a primary care clinic at the New Horizons for New Hampshire Shelter and at the Families in Transition Housing program.


Harbor Care Health and Wellness Center in New Hampshire offers primary, preventive and behavioral health care to youth and adults who are without health insurance, with health insurance, homeless or at risk, living with friends and family, low income or with Medicaid or Medicare
